Common Issues and Resolutions


1. SMS Code Not Received

Possible causes

  • Incorrect phone number

  • Mobile carrier filtering

  • Device has no signal

  • SMS blocked by handset

Resolution

  • Confirm the phone number in Okta is correct (including country code).

  • Ask the user to check signal and SMS capability.

  • Ensure SMS is not blocked or filtered.

  • Attempt to resend the code.

  • If persistent, re-enrol the SMS factor.


2. Okta Verify Code Not Working

Possible causes

  • Device time out of sync

  • User entering an expired code

  • App not fully activated

Resolution

  • Ensure automatic date and time is enabled on the device.

  • Ask the user to generate a new code and enter it immediately.

  • Re-activate Okta Verify if necessary.

Time drift is one of the most common causes of invalid TOTP codes.


3. Okta Verify Push Notification Not Received

  • No internet connection

  • Push notifications disabled

  • Device battery optimisation blocking background activity

Resolution

  • Confirm device has mobile data or Wi-Fi.

  • Check notification permissions for Okta Verify.

  • Disable aggressive battery optimisation for the app.

4. New or Replaced Device

If a user changes or resets their phone:

  • Existing Okta Verify enrolment will no longer work.

  • MFA must be reset or re-enrolled.

  • An administrator resets the user’s MFA factors.

  • User re-enrols Okta Verify or SMS factor during next login.


5. Activation QR Code Not Working
